Hospital at Home (HaH) is a program Island Health will be testing. Through this program, patients who would normally be in hospital will be provided care remotely and in-person in the patient’s own home. Healthcare professionals will be able to provide medications, monitor vital signs, conduct medical imaging and deliver nursing care through a mix of in-person and virtual approaches. This program would allow patients who need care to receive it at home with the support of their caregivers, thereby, minimizing their exposure to viruses such as COVID-19 and others. This program aims to reduce the risk of hospital-driven infections in patients receiving care.

Listening and learning from patients, family care givers, and healthcare professionals is an important part of ensuring this program meets everyone’s needs. We want to hear from you about what you think will be important for us to consider as we launch this testing phase of the program.
